China Deploys Anal Swabs for Covid-19 Testing, Says It's More Accurate

Health authorities in China have deployed anal swab tests to detect covid-19 in the lead up to the Lunar New Year celebrations, according to the Chinese state media outlet the Global Times, a technique that some claim might be more accurate than traditional nasal swabs, throat swabs, and antibody tests. But not everyone is convinced anal swabs are necessarily better.

The Global Times cites Li Tongzeng at Beijing You’an Hospital, a proponent of anal swabs who claims that “the coronavirus survives longer in the anus or excrement than those taken from upper body tracts,” noting the anal swabs are only being taken in a relatively limited capacity.

But the method might be a little more common than Li is letting on. Anal swabs are being deployed in Beijing quarantine as well as in some school settings, according to recent reports out of Beijing’s Daxing district.

From the South China Morning Post:

    The mass testing was conducted after the capital went into partial lockdown in Daxing and Shunyi, with genetic sequencing revealing two cases of the more transmissible coronavirus variant discovered last month in Britain.

    The capital has since been on high alert.

    More than 1,200 people were tested at a school attended by one pupil with an asymptomatic case of the British strain. Each of the contacts at the school had nasal, saliva and anal swabs as well as serum tests – all of which were negative.

Needless to say, an anal swab isn’t as convenient as a throat swab or a nasal swab, and there are no reports of other countries that are prioritizing anal swabs for covid-19 tests. The jury is still out on whether anal swabs, which reportedly take about 10 seconds, are more useful than a regular throat swab to test for coronavirus.

ABC News in Australia reports the “invasive” new test is being used outside of Beijing in places like the city of Guangzhou, where one social media user reported her anal swab was administer on her 14th day of quarantine. She also got an oral swab, according to the news outlet. Another social media user in Beijing said the anal swab test made her feel ashamed.

“Just endless shame. No other feelings. Good luck,” the student, reportedly from South Korea, wrote about the swab in China.

But it’s not clear how much good these tests are going to do during the covid-19 pandemic—at least according to experts in other countries who question the necessity of the practice.

“I’m not quite sure what they’re trying to achieve here with all the anal swabs,” Dr. Sanjaya Senanayake told ABC News.

China has seen a recent resurgence of coronavirus in a handful of hotspots, a troubling development as mutant variations from the UK and South Africa have been show to be more contagious. But China is still doing much better than most countries in the world at containing the pandemic, all things considered.

This week the world surpassed the grim milestone of 100 million cases, with over 2.15 million deaths, a likely undercount of the true death toll around the world.

China mula laksana swab dubur kesan Covid-19

BEIJING - Pemerintah China telah mula menggunakan kaedah calitan dubur (anal swab) ke atas individu yang dianggap berisiko tinggi dijangkiti Covid-19.

Sebelum ini, pakar-pakar perubatan China bersetuju, kaedah itu adalah lebih tepat untuk mengesan kehadiran Covid-19 berbanding ujian calitan tekak dan hidung.

Penyiar rasmi CCTV berkata, pegawai kesihatan telah mula mengambil sampel daripada penduduk di zon merah di Beijing pada minggu lalu.

Penduduk yang ditempatkan di pusat-pusat kuarantin turut diarah menjalani prosedur sama.

Doktor kanan di Hospital Youan di Beijing, Li Tongzeng memberitahu CCTV bahawa kaedah calitan dubur dapat meningkatkan kadar pengesanan individu yang dijangkiti virus berkenaan.

Tambahnya, ini kerana kesan virus kekal lebih lama di dubur berbanding di saluran pernafasan.

"Kaedah ini juga membantu mengurangkan hasil ujian positif palsu," katanya.


Bagaimanapun, menurut CCTV, kaedah swab dubur tidak akan dilaksanakan secara meluas seperti kaedah konvensional yang disyorkan Pertubuhan Kesihatan Sedunia (WHO).

Sementara itu, warganet di China memberikan pelbagai reaksi susulan pelaksanaan calitan dubur berkenaan.

Ada yang mengkritik langkah itu dan tidak kurang ada juga yang menjadikannya sebagai bahan jenaka. - Agensi

KUALA LUMPUR: Ujian calitan tekak dan nasopharyngeal melalui saluran hidung sudah memadai untuk melakukan pengesanan COVID-19.

Pakar virologi dari Universiti Sains Malaysia (USM), Prof Madya Dr Yahya Mat Arip, berkata ia berdasarkan data yang menunjukkan kadar atau peratusan pengecaman virus SARS-CoV-2 dengan menggunakan dua kaedah calitan itu lebih tinggi.

Sehubungan itu, beliau berpendapat tidak perlu ujian pengesanan COVID-19 terhadap najis, dubur atau rektum yang cuma mengundang ketidakselesaan dan kesan negatif dalam kalangan masyarakat.

Mengikut data, kadar pengecaman virus SARS-CoV-2 menerusi calitan nasopharyngeal dan calitan tekak adalah 88 peratus. Calitan najis, dubur atau rektum adalah 70 peratus. Calitan konjunktiva iaitu di selaput lutsinar yang meliputi mata putih dan bahagian dalam kelopak mata pula satu peratus. Pengecaman melalui air kencing dan calitan vagina masing-masing kosong peratus.

Berdasarkan kepada kadar (peratus) pengecaman, ujian calitan nasopharyngeal dan tekak sudah memadai untuk tujuan penyaringan COVID-19. Prosedur yang digunakan juga tidak terlalu membebankan. Ujian calitan najis, dubur, rektum boleh digunakan untuk tujuan saringan COVID-19. Bagaimanapun, prosedur yang perlu digunakan itu akan mengundang ketidakselesaan.

"Ini akan memberikan impak negatif kepada masyarakat dalam usaha menggalakkan masyarakat menjalani ujian saringan. Saya berpendapat kita tidak perlu ujian saringan membabitkan calitan najis, dubur atau rektum," katanya kepada BH, hari ini.

Beliau mengulas ujian calitan dubur di China sebagai kaedah lain mengesan individu dijangkiti COVID-19.

Memetik laporan media, Li Tongzeng, seorang doktor kanan di Hospital You'an Beijing dipetik sebagai kepada CCTV, teknik calitan di dubur boleh meningkatkan kadar pengesanan dalam kalangan mereka yang dijangkiti kerana virus berkenaan bertahan lebih lama di bahagian itu berbanding saluran pernafasan.

Mengulas hal itu, Dr Yahya menegaskan pertama sekali orang ramai perlu membezakan antara 'infectivity' (jangkitan) dengan 'positivity' (positif).

Jelasnya, seseorang yang melalui pengalaman COVID-19 berkemungkinan masih boleh memberikan keputusan positif tetapi tidak berjangkit.

"Yang perlu diberi perhatian adalah keadaan 'infectiousness period' (tempoh berjangkit) yang merujuk kepada 'viral load' (beban virus) yang tinggi dan virus aktif bereplikasi atau dalam erti kata lain mengakibatkan kuantiti virus yang tinggi. Dalam keadaan ini, kebolehjangkitan adalah sangat tinggi.

"Apabila seseorang sudah pulih, masih terdapat kemungkinan saki baki virus yang telah mengalami degridasi atau bermaksud tidak lagi infectious (berjangkit) dalam badan. Ini akan masih memberikan keputusan positif apabila saringan dibuat tetapi kebolehjangkitan adalah hampir tiada," katanya.

Pakar epidemiologi dari Universiti Malaya (UM), Prof Datuk Dr Awang Bulgiba Awang Mahmud, berkata keputusan positif untuk sampel dari sistem pencernaan sudah lama diketahui.

Kajian berkaitan perkara itu sudah setahun dijalankan dan mendapati sampel yang positif dari najis boleh didapati positif lebih lama berbanding sampel daripada sistem pernafasan.

"Misalnya, sampel dari najis boleh positif hingga berpuluh-puluh hari. Bagaimanapun persoalannya, adakah pesakit yang positif calitan dubur masih boleh menjangkiti orang lain? Yang itu perlu jawapan dahulu kerana setakat ini kita tidak mempunyai pengetahuan lengkap mengenai hal ini," katanya.
