They are young, they speak well
Doctors offering rapid HIV test say they see more 'Shenton Way' types

THE uncle who visits Batam frequently or the guy from Geylang's alleys.

You would have thought these were the people who would be making use of anonymous HIV tests offered by two private clinics here.

But doctors at the two authorised clinics say many who go for the test are 'Shenton Way types' - English-educated professionals between 20 and 40 years old.

In June, the Health Sciences Authority approved the 20-minute OraQuick Advance Rapid HIV Antibody Test as a rapid HIV test kit.

The ministry then gave the nod for the two clinics to conduct the tests.

YOUNG PROFESSIONALS

Doctors at the clinics say many of these young working professionals go to them after they learnt about anonymous testing through the Net and elsewhere.

Dr Chua Thiam Eng of Cambridge Clinic in Chinatown said 70 per cent of those he tested were from this group. He has seen 140 patients so far.

Dr Joyce Liang, a general practitioner at Anteh Dispensary in Geylang, said about 30 per cent of those she saw are executive types.

'They are very well-spoken and often come close to lunch time or just before I close at 5.30pm,' Dr Liang said.

Others who go for the test include students and blue-collar workers.

Of course, the doctors do not know exactly what work their patients do since the test is anonymous.

Since June, when the Government allowed anonymous testing, more than 200 people have been tested.

Today, an average of eight tests are done a week by both clinics.

About 80 per cent of those tested are Chinese, 7 per cent are Indians, 5 per cent Caucasian, 2 per cent Malays and about 6 per cent 'others'.

The majority are Singaporeans or permanent residents (81 per cent) and male (89 per cent).

Some go for the test because they have had multiple sex partners, others because they are ready to start a family. Of those tested, 61 per cent are single.

One frightening finding by the doctors is that not many of those who test positive return to the clinic to take the confirmation test.

Dr Liang said she has found three to four positives, but none of them has taken the confirmation test.

The OraQuick test, which takes saliva samples, has a 0.4 per cent, or one in 250, chance of giving a false positive.

Does she keep in contact with the people who tested positive?

'No, there is no way for me to do that,' she said. 'I tell them about the chance of it being a false positive and encourage them to return.'

She added that those who go for the test are usually the more socially-responsible ones.

But she asked: 'What about those who are still out there? What if this is just the tip of the iceberg?'

Dr Liang, who has been practising for 15 years in her Geylang clinic, said that before anonymous testing was possible, she had detected two cases of HIV.

IGNORANCE IS DISASTER

Dr Chua of Cambridge Clinic said: 'A lot of people are fighting this dilemma, 'Should I test and risk finding out that I am positive? Or I just stay ignorant - ignorance is bliss, right?'

'But I tell them, ignorance is not bliss, it is disaster. Getting tested helps you to live your life responsibly.'

Dr Tan Sze Wee, who heads Rockeby Biomed, which distributes OraQuick, said he is buoyed by the public's response to anonymous HIV testing.

'We look forward to the expansion of anonymous testing in more clinics with OraQuick,' he said.

Another anonymous HIV test, which involves a blood test rather than oral fluids, is available at the DSC Clinic in Kelantan Lane near Jalan Besar. (See report on facing page.)

In Singapore, 2,852 people, including 25 children are infected with HIV and 1,547 have Aids. A majority of those infected are men.

--------------------------------------------------------------------------------
Tests use saliva or blood

ORAQUICK works by detecting the HIV antibodies that develop in an infected person after about three months.

A swab is taken from a person's gums because this area contains capillaries that secrete the antibodies.

If the OraQuick HIV result is positive, the person has to do another test called the Western Blot to rule out false positives.

To do this, the person's blood sample will be sent to a laboratory with a code, and the results will be returned to the doctor with the same code - the patient stays anonymous throughout the process.

The number of new HIV/Aids cases in Singapore this year could hit a record high of 340, compared with 317 new cases last year. There has been an average of about 29 new cases a month this year.

ANOTHER TEST
Another rapid anonymous HIV test, which involves blood rather than oral fluids, is available at the DSC Clinic in Kelantan Lane.

Volunteer group Action for Aids says it has done such tests since 1991.

Its spokesman, Mr Lionel Lee, says the finger is pricked for blood and clients get their results in 15 to 20 minutes.

As of November, some 5,400 people have gone for tests there this year.

--------------------------------------------------------------------------------
Health checks to include HIV screening?

THE next time you go for a regular health check, you could be automatically tested for HIV/Aids.

That is if the Ministry of Health (MOH) decides to make HIV/Aids screening an opt-out routine test, reported Channel NewsAsia.

The ministry is now studying the feasibility of this measure in an aim to prevent further transmission by untested HIV-positive people.

And there are figures to back up such fears.

According to the World Health Organisation and UNAids, a United Nations joint programme on HIV/Aids, there are three HIV-infected patients for each one detected. Given that there are about 3,000 HIV patients in Singapore, this could mean a figure of around 12,000 HIV-positive people.

But there are some issues to deal with such as whether the MOH should subsidise the HIV-screening, which costs around $18, or how regularly one should be screened.
