Recurrent fever of unknown origin....

i need a advice la to advice my sis.my sis start demam betul2 after raya ari tuh..quite high gak..39 gitu...then about a week mcm reda..but sbb almost a week gaks then we brought her to hospital kot ada dengue..dah check takde.dia btk gaks..jenih berkahak..kuat gak batuknyer.sometimes she said ada mcm sakit perut gitu gak.
then dmm tuh mcm on off jerk til now.means dah sebulan la..last 2 weeks dia g panel..panel tuh ckp demam bese jek..kasik ubat demam,antibiotik,ubat batuk...then last week demam lgk skali. g klinik lain..still got cough n the dr tkt dia kene tb..so buat x-ray..clear je lungs...
so apa nasihat korang ek?aku pasan batuk dia yg kuat lgk..iyer ler tensen gak dia..dah sebulan bdn tak sihat jek...nih kene outstation lak..apa kebarangkalian yg ada yer?or it just her body lom dpt get rid of the previous infections?

Please rule out Pulmonary Tuberculosis(PTB), especially for long standing, on & off fever + loss of appetite, loss of weight.
SImple test, can be done at GP:
      - Sputum sample early morning for 3 consecutive days(for acid fast Bacillus staining)
      - Chest X-ray : not all PTB, especially early will show up the classical 'cavitation sign on CXR.
                           if the doc say NO PROBLEM, CLEAR CXR, but u need 2nd opinion, u can bring the CXR to the Radiologist(CXR Reporting)

Pneumonia could also be the possible cause. Blood culture help to rule out pneumonia. Again CXR may seem normal to the eye of Non-Radiologist.
If extensive investigation has been carried out(blood culture, sputum sample, CXR Reporting, UFEME), and your sis still hjaving the on & off fever with cough, then only it could be
pyrexia(fever) of unknown origin.

alamak..i don't think that she's bringing the x-ray to kk.rsnyer yg sputum tuh dia takde buat rsnyer.then yg blood test tuh ada dia buat after seminggu demam..check for dengue.then the other one few days later after demam tak kebah gaks at another hospital.they said takde kuman dlm darah...ada tak chances kuman tuh akan ada dlm darah skang n she probably need another test?

Hmm..so the initial blood cultures were Negative.
My opinion, within this week if she still have the swinging fever, go for both blood culture(again) and sputum sampling.
Don't worry so much for the CXR for the time being.

reddevils..thanks i just ask her to check the sputum if she doesn't get well by this week.i just chit chat with my pren yg wifenyer penah kene tb..rupernyer tb not just attack lungs ek..like her wife dia attack kelenjar bwh ketiak...

general people usually think TB is a lung infection. Yes, it's true. But u should also know that TB can also infect:
-brain
-bone and joints

aiyark not yet...i've tell her to do so..maybe lom lgk dibuatnya coz dia br sampai kk last monday...br nak menyesuaikan diri...dia nih i rs daya kerintangan penyakitnya rendah skit.coz last year b4 she went to sudan she was coughing alot n transmit the disease to my son n he end up being admitted. yg dia pun balik dr sudan demam2 plak..risau gak i..trus bwk g spital..admitted doc said maybe viral infection...risau gak i sbb time tuh sbb dia amik shots pun seme last minutes...

i just called my sis last nite.she had no more fever.and her cough is almost suppressed now.look like udara sabah is better for her...
